I am involved in a project aimed at developing a stick-on skin sensor patch for the acoustic detection of coronary artery stenosis for which in-vitro models of the main anatomical structures in the chest are necessary. This work, originally funded by an EPSRC project grant, started with a crude model set-up consisting only of a piezo-electric vibrator to simulate the disturbed/turbulent flow associated with a stenosed artery and a detector (microphone/accelerometer) between which were positioned various soft tissue-mimicking gels. Shear and transverse waves were generated and detected in this system and their propagation, modelled numerically. This simple physical model has progressed to a crude chest phantom consisting of a latex ‘coronary artery’ embedded in a soft-tissue mimicking gel fitted with stenoses of various geometries, surrounded by a polyurethane “skin”, through which fluid is pumped. The detector has now progressed to an adhesive patch containing 8 accelerometer/microphone pairs, attached to the model’s skin and the complex signals obtained show that disturbed flow can be detected. We have funding for a small clinical trial, involving simultaneous patch and angiographic measurements, pending ethics and MHRA approval, but the data generated from patients is much more complex than that produced by our current phantom. On the basis of preliminary work into the feasibility of using a machine learning approach to determine, not only the presence or absence of a stenosis but also its location and severity, we have established that a K-Nearest-Neighbours and Random-Forest ensemble approach works with simple in-silico data. However, a more realistic model incorporating a moving heart, lungs and bone, plus coronary arteries is needed to provide data in sufficient quantity to provide an adequate training and testing environment for the ML algorithms and to augment the obviously more limited in-vivo data from patients with and without occlusive arterial disease.
